Medicare IRMAA adds $81.20 to $487 a month to Part B in 2026
IRMAA is an extra charge on Medicare premiums for people with higher incomes. In 2026 it starts at single income over $109,000 or joint income over $218,000. It adds $81.20 to $487 a month to Part B and $14.50 to $91 a month to Part D.

Income means modified adjusted gross income. The Part B add-on is on top of the $202.90 standard premium. The Part D add-on is on top of your plan's premium. What this price covers
The Part B add-on has applied since 2007 and the Part D add-on since 2011. Both are paid to Medicare, even if your Part D premium goes to a private plan.
What changes your price
- Income. The add-on rises in five steps.
- Tax filing status. Married filing separately reaches the high brackets fastest.
Ways to pay less
- If your income fell because of a life event such as retirement, ask Social Security to review your IRMAA.
- Watch the bracket edges. One dollar over can mean a higher add-on all year.
